On July 14, 2024, the Honolulu Fire Department (HFD) received a 911 call at 4:34 p.m. and responded to a motor vehicle collision near 66-945 Kaukonahua Road in Waialua with 4 units staffed with 14 personnel. The first unit arrived on scene at 4:42 p.m. and reported a two-vehicle accident with two occupants trapped in their vehicles. A third occupant was found dead on arrival.

HFD personnel stabilized the vehicles and used hydraulic rescue tools to extricate the two occupants and transferred medical care to the Emergency Medical Services at 4:57 p.m. HFD personnel secured the scene and transferred command to the Honolulu Police Department (HPD).

All HFD personnel were accounted for with no injuries.
