UPDATE: The Honolulu Fire Department (HFD) Fire Investigators have classified the cause of the fire that occurred on January 6, 2025, at 1645 Young Street in Honolulu, where Fire Fighter Jeffrey Fiala tragically lost his life, as accidental due to unattended cooking.

The HFD in collaboration with the Bureau of Alcohol, Tobacco, Firearms and Explosives National Response Team completed the on-scene origin and cause fire investigation. They determined the fire originated in the kitchen, located at the rear of the structure.

Four other fire fighters and an occupant were injured and hospitalized. As of today, only the occupant remains in the hospital.
