On September 21, 2025, the Honolulu Fire Department (HFD) received a 911 call at 9:55 a.m. and responded to a sick hiker on the Diamond Head Trail in Kahala with 6 units staffed with 17 personnel. The first unit arrived on scene at 9:58 a.m., established command and ascended the trail on foot. The next-arriving unit secured a landing zone nearby to prepare for air operations.
It was reported that a 65-year-old female was hiking alone, became dizzy and could not continue on her own. HFD personnel on foot arrived at her side at 10:15 a.m., performed a medical assessment, provided basic life support treatment, and prepared her for extraction. Air 1 inserted a rescue person who packaged the hiker and the air team hoisted her safely aboard the aircraft. Air 1 transported the hiker to the nearby landing zone, where her medical care was transferred to the Emergency Medical Services at 10:59 a.m.
All HFD personnel were accounted for with no reports of injury.
