On May 25, 2026, the Honolulu Fire Department (HFD) received a 911 call at 5:42 p.m. and responded to an injured hiker on the Koko Crater Trail with 6 units staffed with 17 personnel. The first unit arrived on scene at 5:54 p.m. and ascended the trail on foot. Additional units established command and secured a nearby landing zone to prepare for air operations.
It was reported that a female in her 20s suffered a medical episode. She could no longer continue on her own to descend from the trail. Rescuers on foot arrived at her side at 6:11 p.m., performed an initial medical assessment, provided basic life support treatment, and prepared her for extraction. An additional Rescue personnel unit arrived at the hikers side to aid with air extraction preparations. Air 1 arrived to the rescuers and performed the extraction. Air 1 transported the hiker to the nearby landing zone, where medical care was transferred to the Emergency Medical Services at 6:44 p.m.
All HFD personnel were accounted for with no injuries.
